Biliary stents in malignant obstructive jaundice due to pancreatic carcinoma: a cost- effectiveness analysis

چکیده
Health technology The use of two methods for endoscopic placement of biliary stents in patients with pancreatic carcinoma. The methods examined were plastic (polyethylene) stents and metallic stents. Plastic stents were widely used due to their ease of insertion and low costs. However, their major disadvantage was their tendency to clog because of bacterial biofilm formation and biliary sludge. Metallic stents were associated with lower occlusion rates. However, their major disadvantages were the high costs and the difficulty of repositioning or extraction once deployed.

Management of the mucin filled bile duct. A complication of intraductal papillary mucinous tumor of the pancreas.
CONTEXT Biliary obstruction secondary to intrabiliary mucin is a relatively rare presentation of malignant intraductal pancreatic mucinous tumor. We report a case of unresectable intraductal pancreatic mucinous tumor associated with obstructive jaundice secondary to intrabiliary mucin. This case and a review of the literature, highlight the difficulty in obtaining sustained palliation from jaun...

167 misconceptions are true today. The results of the study being reviewed emphasises the importance of such patients being referred to major centers. In my opinion the morbidity rates can be improved significantly even though most of the complications were minor. Operative mortality should be below 5% in expert hands. When one notes that 30% of the patients with solitary tumours and as many as...
متن کاملEndoscopic palliation for inoperable pancreatic cancer.
BACKGROUND The majority of patients with pancreatic cancer are not candidates for surgical resection. Palliative therapy remains the cornerstone of management of this population. METHODS We reviewed recent clinical and experimental studies on endoscopic palliative therapy of inoperable pancreatic cancer. RESULTS Endoscopic placement of a biliary stent is the preferred mode of palliation of ...
متن کاملStenting and interventional radiology for obstructive jaundice in patients with unresectable biliary tract carcinomas
Together with biliary drainage, which is an appropriate procedure for unresectable biliary cancer, biliary stent placement is used to improve symptoms associated with jaundice. Owing to investigations comparing percutaneous transhepatic biliary drainage (PTBD), surgical drainage, and endoscopic drainage, many types of stents are now available that can be placed endoscopically. The stents used a...
متن کاملSurgical Bypass versus Endoscopic Stenting for Unresectable Head Pancreatic Cancer, Which Palliative Treatment Is Better in Developing Countries, Morocco as an Example
Background: Metal stents for unresectable pancreatic cancer are associated with longer patency and superior cost-effectiveness. However, they are too expensive to be recommended routinely in developing countries. Moreover, a debate on outcome results in these patients who receive plastic biliary endoprothesis versus surgical bypass as palliation of obstructive jaundice. We aimed to compare retr...
